Must have a BA in Health related areas, i.e. Social Work, Sociology, Psychology
Social Worker Assistant responsibilities:
- Assessments of participants regarding social and emotional factors affecting health problems. Formation of individual plan of care.
- Discharge planning services.
- Providing referral.
- Liaison with participant's family and caregivers
- Care management.
- Cantonese speaking preferred.

Pay: $18.00 - $21.00 per hour
